The American Medical Association (AMA) has created three new CPT® codes to report the administration of the Moderna COVID-19 vaccine for patients 6 through 11 years. The new codes are effective June 17, 2022.

Note These New Moderna Admin Codes

For children ages 6 through 11 years, report the Moderna vaccine with existing CPT® code 91309 Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2 (coronavirus disease [COVID-19]) vaccine, mRNA-LNP, spike protein, preservative free, 50 mcg/0.5 mL dosage, for intramuscular use and one of the following new admin codes:

0091A Immunization administration by intramuscular injection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (coronavirus disease [COVID-19]) vaccine, mRNA-LNP, spike protein, preservative free, 50 mcg/0.5 mL dosage; first dose, when administered to individuals 6 through 11 years

0092A second dose, when administered to individuals 6 through 11 years

0093A third dose, when administered to individuals 6 through 11 years

The Moderna vaccine product reported with 91309 comes in multiple-dose vials and features dark blue caps and labels marked with teal borders.

Per CPT® Assistant Special Edition: July Update, report 91309 with admin codes 0091A, 0092A, 0093A, and 0094A only. Note that 0094A is not new but is revised, effective June 17, 2022, to specify that the vaccine reported with 91309 is a booster dose when administered to individuals 18 years and over.

Moderna Vaccine Codes for Ages 6 Months to 5 Years

Moderna’s COVID-19 vaccine received emergency use authorization (EUA) by the Food and Drug Administration (FDA) for patients 6 months to 5 years, effective June 17, 2022. Three new CPT® codes (0111A, 0112A, and 0113A) became effective the same day.
